I am trying to implement some sort of a customer satisfaction survey on our site. Anyone has any suggestions for an email based quick survey that works with MIVA?

I am actually using this module.
I guess what I am trying to say is: How would I capture this information? Ideally I would like this to be a link within the follow up email that takes customer to a short online submission form.

So you would just apply your survey link to the follow-on contact email template, then if you want MIVA integration, use one of the contact form modules. (take a look at the one from ADS (http://www.mivacentral.com/Merchant2/merchant.mv?store_code=m&screen=PROD&product_code=STL-CONTACTFORM))

There are several php scripts out there that do this and they can be called in. If you use Scot's Mailing List (http://www.designextend.com/page/miva/PROD/scotmail) you could use it's custom fields to request some responses and maintain those in a separate list.

However, if you're looking for a true survey script, you'll need to look on the outside. There are also some services offering this, but I've found most tend to be quite pricey for us smaller guys.
