Adding a text box

I was wondering if anyone knew how to add a "how did you hear about us" box and "additional questions/comments" text box to the OCST Checkout;customer information page?? Ideally this information would appear on the invoice page.

I have addendum and it is a great module...but if you implement Google checkout you lose the function for any sales completed thru google.

I created a google checkout note product with $0.00 price and 0 weight, that I link to from the basket page in front of the google checkout button and added a text box attribute to this product. It is kind clunky I will admit, but it at least allows for a comment to be added.
