Online Survey Service

Can anyone recommend an online survey service. We want our customers to fill out an online survey, so we can then analyze the answers.

The questions would be multiple choice, with an "other" option. At the end, they would get a coupon.

Any suggestions would be helpful!

Don't know if you have an email newsletter solution, but OemPro (http://www.jmonline.net/oempro) has a plugin called SurveyManager that is excellent. New version about to be released with new features too.
