Why do a certain standard set of categories show up in the category tree on most of my pages, but additional categories show up on other pages? Where do I tell Miva to display/not display a category?

Do you have an example (paste URLs, please).
If you're referring to subcategories, only those that apply to the parent category screen you're on will appear.
When viewing a category that has sub-categories set to it (or viewing one of its sub-categories) , all subcategories will now show on the tree.
So if you have an inactive parent category then normaly none of the subcategories will show, but as soon as you go to one of those subcategories - all other subcategories of that parent will show.

Is there any way to avoid this? I suppose make the subcategories their own main category...

Not by default. If you want all subcategories to display, then they either need to be their own categories, or you'll need to create your own tree via HTML.
