Emailing a Customer his/her LOGIN

Hi all new forum users!

I have a question regarding emailing lost login names. We currently have our site setup to email a customer if he/she has lost his/her password. But what if they've lost their login name? How can I set it up so they can be emailed their login name and/or password?

Sebenza's Login Lookup (http://www.mivacentral.com/Merchant2/merchant.mv?Screen=PROD&Store_Code=m&Product_Code=SS-LOOKUP&ref=WPAW) will allow the customer to look up their login and password by their email address.
