When I run the wizard for Set Up Payment, it doesn't list Cybersource ICSCv2. I have the modules setup, and several other payment types are listed.
According to the documentation :
...
4. Select Set Up Payment
5. Click Other
6. Select CyberSource ICSv2 Payment Services
I still haven't found the other to click on either.

What version of Miva Merchant are you running?

Wow, guess they got the new forum topics going already..
You dont see Cybersource because your host has not installed the ICS commerce library.
Unlike previous versions of Merchant, Merchant v5 will only show payment options that are installed.
Ask your host to install the ICSv2 commerce library.

I did copy the library files over to the lib/commerce directory on my server.
Miva did the original install, so I'm not sure if they used the vinstall my server has to install Miva.
Also, according to the documentation for Miva, there are some changes to httpd.conf. I don't see any of those changes in the file, but other commerce payment types are listed.

We're using 5.03.

Then you will need to modify your httpd.conf to add the ICSv2 commerce library path just like the existing other commerce libraries.

Vic is correct that you need to have the Commerce Library installed for any of the Payment Modules to show up in Payment Configuration or in the Set Up Payment Wizard.
If the ICS2 doesn't show in the list, then it is absolutely the Commerce Library not installed.
BTW, I made the 5 Docs, so I am so excited to see that you are using them! YAY! Validation...

Now if we can just get some co-operation between MIVA Support and Cybersource's support.
Anytime you call, its the other guys fault.
When you partner with a company to provide a bundled or intergrated service, make sure both sides tech support has the ability to troubleshoot from both ends.
Its not fair to end users to be pushed around from one company to the other to find answers. They are already frustrated and upset.
Since MIVA brands this as MIVA Payment, I think it would be in the best interests of all to at least provide first level troubleshooting for the Cybersource product...as well teach the Cybersource folks how to read, interpret and troubleshoot some of the MIVA error messages.
At least give them some docs!!!

I am updating the Documentation to reflect the Commerce Library issue today and I am glad that this was brought up as it enables me to correct the information. I will fix it this afternoon.
As for support, I am sorry you have had such a difficult time and I will forward your comments over to our Support Supervisor.
